The purpose of this program is to provide an overview of how to develop a patient navigation program in various practice settings and share best practice models. After attending this webinar, participants will be able to —
- Define building blocks to successfully develop, implement and sustain the role of the nurse navigator in a community cancer program.
- Identify how the nurse navigator contributes to oncology clinical service line, quality outcome measurements, retention and downstream financial revenue.
- Explain an example of a best practice patient navigation model in a community cancer program.
