The goal of this webinar is to empower nurses with knowledge in the continuum of care and to learn of the opportunities for multidisciplinary efforts to develop transitional care systems which will deliver better quality healthcare and outcomes. Upon completion, participants will be able to:
  • Identify the top three reasons for the need of a transitional care nurse.
  • Define the importance of a seamless transfer of patients’ health information across the continuum of care and healthcare systems
  • Discuss patient centered care that bridges the gaps in achieving quality of care during transition
